1023 Questionnaire for Charitable Organization

We are preparing your 1023 form, in which you ask the IRS to recognize your organization as a 501c3 charity.

In order to fill this form, there is some information we need to pull together. It is easiest for you if you speak to us by calling 561-212-1192 and giving the information. However, there are many people who prefer to answer in writing. Answering in writing actually helps speed up the process.

1) History and Activities:

Organization Name(Required)

a) How did your organization get started? What is the date of your corporation’s filing with the state? Has your corporation been in existence for over 1 tax year, or less than that?

b) What you have done and what you plan to do in the future?

c) What is the main activity or activities your corporation does?

d) Who conducts the activity?

e) Where is the activity conducted?

f) What percentage of your total time is allocated to the activity?

g) How is the activity funded (for example, donations, fees, etc.)?

h) What percentage of your overall expenses is allocated to this activity?

2) Directors and Special Expenditures:

a) Do you or any other of the directors of officers intend to get paid?

b) What will they be paid for?

c) How much will they be paid?

d) Please send me the emails of your directors. This will allow us to send out your bylaws and other documents for e-signing.

4) Financial Information:

a) When was your corporation started?

b) How much money are you expecting to make in the coming year?

c) How will you make the money? Donations? Membership fees? Tuition?

d) Will you be selling anything?

e) How do you expect to spend the money?

f) Will you be selling anything?

g) How do you expect to spend the money?

Since you are seeking recognition as a school, you need to tell us about your plans:

About the school:

a) How many children are you planning to enroll in the fall of 2024? What grades will they be in?

b) How many teachers, administrators, teachers’ assistants will you be hiring?

c) How many general subjects’ teachers? How many Religious subject teachers? (we have lots of clients who are religious parochial schools)

d) How many support staff (janitorial/custodial) Nurses? Counselors?

e) How much do you expect to be paying the staff?

f) What do you expect the racial make up of the student body will be?

g) How about the staff? The teachers? The general studies teachers?

h) Will you be providing scholarships of tuition discounts to parents of students. What criteria will you follow in approving requests for scholarships?

i) Where will your school be operating. Rent or buy?

If you have been in business already, please supply similar information going back in time. (If you have been in business for a long time, you need to go back in time 5 years, but no more)
